Do you have new patient paperwork?

Once you have scheduled your new patient appointment online, you will receive an email to access your online intake paperwork. Please complete all required documents for your specified appointment prior to arriving at the office for your first visit. Should you decide to fill out your paperwork in the office prior to your appointment, please arrive 15-20 minutes early.

Do you accept my insurance?

In order to better serve our patients with comprehensive and inexpensive health care, we are not currently in-network with insurance providers. This allows our patients to avoid the hassles of co-pays, deductibles, non-covered services, and visit restrictions so they can receive the top quality care. We accept cash, check, and debit/credit cards.

Additionally, we are happy to print a ‘superbill’ upon request. As the patient, you can submit this document to your insurance company for potential reimbursement for any covered services under your provider plan.

What can I expect at my first visit?

Each new patient appointment includes a comprehensive history and physical examination to determine the extent of care required to restore health and improve function. Detailed care plans are developed and specifically designed to fit each patient’s needs and healthcare goals.
